homework problem

Can anyone fill this out and show me the formulas used to arrive at the answer.  I keep putting it in but getting incorrect, I don’t know what I’m doing incorrectly.  I need it TODAY, but again I need the answers showing how you arrived at each…I’m doing something wrong but don’t knwo what it is.
